UNDER NO CIRCUMSTANCES SHALL FOUNDATION BE LIABLE FOR ANY DIRECT OR INDIRECT, SPECIAL, INCIDENTAL OR CONSEQUENTIAL
DAMAGES THAT MAY ARISE FROM A CLAIM OF BREACH OF THIS PRIVACY POLICY. Some jurisdictions do not allow the exclusion or limitation
of liability for consequential or incidental damages. In such jurisdictions, our liability is limited to the fullest extent permitted by law.
PROCESS FOR REVIEW AND CHANGES
Given the limited information collected and the form in which Foundation receives such information, Foundation does not offer a process for
review and changes to personal information.
YOUR CALIFORNIA PRIVACY RIGHTS
You have the right to request that Foundation disclose to you what information it collects, uses, discloses, and sells. You also have the right to
request that a business delete any personal information about you which the business has collected about you. To submit either of the above

sent to Foundation to delete your personal information will be subject to our verification procedures, including verifying the identity of the

Foundation shall not discriminate against you for exercising any of your above rights.
You may designate an authorized agent to make a request to exercise your rights on your behalf. For your protection, we reserve the right to deny any request from an agent who does not submit proof that they have been authorized to act on your behalf.
Foundation has no actual knowledge that it sells personal information of minors under age 16.

Under California’s “Shine the Light” law, a California resident with whom Foundation has an established relationship has the right to request
information about Foundation’s disclosure of personal information to affiliated companies or third parties for their direct marketing purposes,

YOUR NEVADA PRIVACY RIGHTS
Nevada residents may submit a verified request not to sell their personal information by contacting us by writing to us at

CHILDREN AND MINORS
Foundation makes no active effort to collect personal information from children under the age of 18 and does not wish to receive any such
